I have searched and searched and cannot find the IMAC sign in sheet that is referenced in the CD's contest checklist. Anyone know where it is?

And I am not sure you will. What i have done in the past is cut and past the pilots sign up here on the IMAC into an Excel spread sheet as all the info you want is right there, and it will allow you to add info you would like to collect such as email address' and such.

Yep.. I made my own in EXCEL...

I think I found a printed out one too.
